We refer to our previous communication with regards to Precision Medicine Coverage Memo dated 20 September 2021.
Cancer Precision Medicine (cancer genomic test and targeted therapy) is covered by PruBSN since 6 September 2021. Please be reminded that Hospital Alliance Service (HAS) facility is applicable for admissions which required for the genomic test and targeted therapy. As per usual practice, the treating doctors may request for Pre-Authorization Letter (PAL).
To ease our HAS assessment, we wish to inform that all our panel hospitals are required to submit Genomic Test – Lab Order Form of any laboratory partner of each hospital during PAL request. The genomic test – lab order form will clearly provide information of type of genomic being requested to test and will reduce unnecessary deferments.
* IN SUMMARY, the REQUIRED DOCUMENTS to submit to PruBSN for admissions with arrangement of genomic test are :
- PruBSN’s Pre-Authorisation Form – Part 1 completed by Person Covered and/ or Certificate Owner
- Pre-Authorisation Form – Part 2 (Admission Section) completed by Attending Doctor (kindly ensure no questions are left empty)
- Copy of patient’s NRIC (or any proof of identity)
- Any readily available reports (including HPE report) or referral letters
- Any readily available Covid-19 test results
- Copy of genomic test – lab order form (any laboratory partner of each hospital) → New Requirement !!!
